The Alliance for Health Equity landscape scan for healthcare and food partnerships in Chicago and suburban Cook County is open and can be filled out from September 24 – October 12, 2018 to share information about programs and initiatives.
All organizations involved in healthcare and food security/access partnerships in Chicago and suburban Cook County are invited to participate in the landscape scan and help build a more complete picture of existing programs in our area and to reveal gaps and system needs. (See below for more information about the landscape scan.) The landscape scan can be completed in two ways.
- GENERAL LINK: If you use this, you will need to complete the scan in one sitting; you may not be able to finish a partially completed scan and/or previous responses may not save. https://uic.ca1.qualtrics.com/jfe/form/SV_5cnzi8er0nXwAjX
- SPECIALIZED LINK for your organization: This option allows you to request a link that automatically saves responses until they are submitted. It allows a participant to complete the scan in multiple sittings and/or partially-complete scan then send the link for other staff within your organization to fill out other sections. This scan is designed to gather one response per organization, and organizations with multiple healthcare and food partnership programs can share information about multiple programs. The work of the Alliance for Health Equity Food Access + Food Security workgroup is guided by a framework and timeline. View the framework and timeline here.
Objectives:
- Inventory healthcare partnerships for food security and food access, partners involved, and their stage of exploration/implementation (based on an implementation science framework)
- Understand program goals and any existing evaluation and/or metrics
- Identify common barriers and challenges and approaches to overcoming those barriers
- Gather information on implementation processes and procedures for screening and referral programs underway
- Understand funding mechanism and sustainability
The information collected through the Landscape Scan will be compiled, analyzed, and reported in conjunction with the collaborative Community Health Needs Assessment (CHNA) process underway. The Alliance for Health Equity workgroup will use the information collected to reveal gaps, system needs, and potential collaborations. A report of the landscape scan findings will be shared with all respondents.
