United to Improve Health Across Chicago & Cook County

For nearly a decade, the Alliance for Health Equity has convened to improve population health and promote health equity.

The Alliance’s collaborative hospital-community partnerships and specialized workgroups strengthen community health outcomes in the City of Chicago and Suburban Cook County. Along with completing the area’s Community Health Needs Assessment (CHNA), we convene workgroups to address the social and structural determinants of health and the increasing need for food, housing, and mental health support. Our work has impacted individuals and families all across our service area by connecting local initiatives and leaning on the knowledge of the true experts - the residents of our communities.

Every individual experience is key to understanding the overall health of our community. We are currently recruiting all Cook County (including Chicago) residents to complete the Community Input Survey, a key component of the CHNA that allows us to hear directly from those most affected by health challenges.

We are conducting our annual Community Health Needs Assessment.

Your input is very important and will help create a plan to improve community health. Your responses are anonymous.

The survey is for people of all ages living in Chicago and Suburban Cook County.
The Community Input Survey should take about 7-10 minutes to complete.

Case Study: Veggie Rx

VeggieRx provides participants with a bag of produce, vouchers - “Produce Perks” - that are redeemable for additional produce at onsite farm stands. Additionally, nutrition lessons and a cooking demonstration using produce that participants purchased with the voucher are available. The cooking demonstration is used to spark discussion on healthy cooking and nutrition.
