Since 2016, the Alliance for Health Equity - a collaboration between the Illinois Public Health Institute and area hospitals, health departments, and community organizations across Chicago and Cook County - has convened to complete the area’s Community Health Needs Assessment. 2022 Community Health Needs Assessment
In 2022, the Alliance for Health Equity completed a collaborative Community Health Needs Assessment (CHNA) for Chicago and Suburban Cook County. The report builds on the previous assessments and allows partners to identify strategic priorities that they can collectively address to improve community health. 2019 Community Health Needs Assessment
In 2019, the Alliance for Health Equity completed a collaborative Community Health Needs Assessment (CHNA) for Chicago and Suburban Cook County. The report builds on the previous assessments and allows partners to identify strategic priorities that they can collectively address to improve community health.
The Patient Protection and Affordable Care Act (ACA) requires every non-profit hospital to conduct Community Health Needs Assessments (CHNA) and implement plans that address identified community health needs. Each member hospital of the Alliance for Health Equity posts an individual hospital CHNA which incorporates data from the collaborative CHNA.
